1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her work environment to control or eliminate any hazards or other exposure to illness or injury: Worksite located at Buck O'Neal Bridge, Kansas City, Missouri-Employees were exposed to struck-by or crushing hazards in that the employees were misusing the lever hoist (come-a-longs). The employees were using the hook improperly, the load being pulled was not properly seated in the base (bowl) of the hook, the load was placed on the tip of the hook, which also caused the hook to become distorted and bent. On or about February 21, 2023, an employee was struck in the head when the lever hoist's hook slipped off the I-beam's stiffener. FEASIBLE AND EFFECTIVE MEANS OF ABATEMENT: 1. Initiate a process of attaching steel plates to the bridge girders so that the shackles can be used to attach the hooks of the bowl or base of the load hooks. 2. Train employees on the safe usage of the lever hoists and implementing the practice of inspecting the equipment prior to usage. 3. Implement a procedure to take hoists out of service if found defective. 4. Do not allow employees to point load hooks for the lever hoists or, use point load hooks that is designed to accommodate point loading. 5. Per ASME B30.21-2014. Chapter 21-2. Inspection and Testing. SECTION 21-2.1: INSPECTION21-2.1.1 General. (a) All inspections shall be performed by a designated person in accordance with the manufacturer's recommendations and with the requirements of this Volume. Any deficiencies identified shall be examined and a determination made by a qualified person as to whether they constitute hazards and whether a more detailed inspection or disassembly is required.

29 CFR 1926.350(a)(11):Cylinders were not stored in a well-protected, well-ventilated, dry location, at least 20 feet (6.1 m) from highly combustible materials such as oil or excelsior: Worksite located at Buck O'Neal Bridge, Kansas City, Missouri- Employees were exposed to a fire or explosion hazard in that oxygen and acetylene bottles were being stored inside a sealed convex box, in which gasses could accumulate and ignite when an ignition source was present.
