29 CFR 1926.1427(b)(4)(i): The employer did not ensure that the operator-in-training was continuously monitored by an operator's trainer while operating the crane equipment: (a)Tull Companies, LLC at 4920 East Rosa Parks Place, Sioux Falls, SD 57110: On or about and at times prior to February 28, 2023, the employer did not ensure that the crane operator-in-training performing lifts with the Link - Belt 90 Ton Hydraulic Crane HTC - 8690,Unit ID: CL190, Serial #: N3J8-0349, was continuously monitored by an individual who met the requirements of 29 CFR 1926.1427(b)(4)(i)(A) - (B). This condition exposed subcontracted employees to struck-by hazards.
