29 CFR  1926.651(c)(2):  A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  Employees engaged in storm water pipe installation at an apartment complex construction located at 11851 Roanoke Blvd, Omaha, Nebraska were exposed to fall hazards in that employees were not provided a safe means of egress.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  This standard is applicable to construction employment as defined by 29 CFR 1926.59.   Employees engaged in storm water pipe installation at 11851 Roanoke Blvd, Omaha, Nebraska were exposed to contact with hazardous materials and/or toxic substances in that employers did not provide adequate information for the on-site specific chemicals and measures that employees can take to protect themselves from these hazards.  Chemicals used on-site include but are not limited to glue and primer.
