29 CFR 1910.28(b)(3)(iv): The employer did not ensure each employee was protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toe boards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used.  a) On or about 2/28/2023 by press # 52; employees were exposed to a fall hazard of approximately 10 feet when the top of the ladderway used to access the stacker mezzanine was open without any protection from falling into the opening at the top of the ladder.   b) On or about 2/28/2023 by press # 52; employees were exposed to a fall hazard of approximately 10 feet when the top of the ladderway used to access the press panel mezzanine was open without any protection from falling into the opening at the top of the ladder.
