On or about April 21, 2023, the employer failed to submit documentation and certification of corrective action for citation 1, item 1 issued on March 2, 2023 with the abatement due date of March 21, 2023.  On or about April 21, 2023, the employer failed to submit documentation and certification of corrective action for citation 1, item 3 issued on March 2, 2023 with the abatement due date of March 21, 2023.

This condition exposed the employee to fall and struck-by hazards.
