29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: On or about February 23, 2023, employees were exposed to falling through a hole in excess of 6-feet above the lower level when removing a piece of metal roof decking from the north side of the building without adequate fall protection.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: On or about February 23, 2023, employees were exposed to fall hazards when attempting to remove a piece of metal roof decking from the north side of the building. The personal arrest system did not stop the employee from falling approximately 21.5 feet to the second level below.
