29 CFR  1910.28(b)(1)(i):(1) Unprotected sides and edges.  (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling.  An employee engaged in maintenance operations at 2401 Production Drive, Roca, Nebraska 68430 was exposed to fall hazards in that an employee was working at heights greater than 4ft without any form of fall protection  while servicing mechanical components on top of the oven.

29 CFR  1910.147(c)(1):Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  An employee engaged in heavy machinery operations at 2401 Production Drive, Roca, Nebraska 68430 was exposed to electrical and pneumatic hazards in that the employer did not establish a LOTO program, LOTO procedures, and periodic LOTO inspections on the star bend machine.

29 CFR  1910.178(l)(2)(ii):Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  Employees engaged in forklift operations at 2401 Production Drive, Roca, Nebraska 68430 were exposed to struck-by hazards in that employees were not trained as required by the standard.

The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  Employees engaged in forklift operations at 2401 Production Drive, Roca, Nebraska 68430, were exposed to struck-by hazards in that employees were not trained and evaluated per OSHA standards.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  Employees engaged in wood working and fabrication operations at 2401 Production Drive, Roca, Nebraska 68430, were exposed to caught it hazards in that employees were using unguarded drill presses in the fabrication and wood working areas of the facility.
