29 CFR  1910.333(a)(1): Live parts to which an employee may be exposed were not deenergized before the employee worked on  them:  (a)5905 Flintridge Drive, Colorado Springs CO., On or about 2/24/2023, and at times prior,  the employer, Pikes Peak Christian School, did not ensure that employees conducting electrical work, deenergized the fuse panel before performing work on it. The fuse panel contained three Bussman FRS-R-60, 60A, Class RK5 Fuses rated at 60 amps/600V which needed to be removed and replaced.  An employee sustained electrical burns to the face, torso, and arms as a result of an arc flash while attempting to remove the fuses while the panel was energized with uninsulated metal vice grip pliers.

29 CFR  1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:   (a) 5905 Flintridge Drive, Colorado Springs CO., On or about 2/24/2023, and at times prior, the employer, Pikes Peak Christian School, exposed employees to electrical hazards. The employer did not train employees assigned to perform electrical work on the safety related work practices necessary to perform the work in a safe manner. A non-qualified employee was attempting to remove a fuse from an energized fuse panel which contained three Bussmann FRS-R-60, 60A, Class RK5 Fuses: rated at 60 amps/600V. The employee sustained electrical burns to the face, torso, and arms as a result of an arc flash while attempting to remove the fuses with uninsulated metal vice grip pliers.

29 CFR  1910.335(a)(1)(i):Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed:  (a) 5905 Flintridge Drive, Colorado Springs CO., On or about 2/24/2023, and at times prior, the employer, Pikes Peak Christian School, did not ensure employees were provided with electrical protective equipment while changing out three (3) energized Bussman FRS-R-60, 60A Fusetron Dual Element Class RK5 fuses, rated at 600V. Employees were exposed to electrical hazards in that an employee sustained electrical burns to his face, torso, and arms, as a result of an arc flash.

29 CFR  1910.335(a)(2)(i):When working near exposed energized conductors or circuit parts, each employee did not use insulated tools or handling equipment when the tools or handling equipment might have made contact with such conductors or parts:  (a) 5905 Flintridge Drive, Colorado Springs CO., On or about 2/24/2023, and at times prior, the employer Pikes Peak Christian School exposed employees to electrical hazards in that no insulated tools were used or provided to employees while attempting to change out three (3) energized Bussmann FRS-R-60, 60A, Class RK5 fuses rated at 60 amps/600V from a fuse box. The fuses were removed with uninsulated metal vice grip pliers resulting in an arc flash. An employee sustained  electrical burns to his face, torso and arms.
