On or about March 1, 2023, and at times prior thereto in the Welding and Fitting production areas, employees were exposed to struck-by hazards in the following instances (a-c): a) At the Fit-up Welding Station - the JLC Coffing 1 Ton Hoist hook was missing its intended safety latch; b) At the Fab Production Saw Area Harrington 2 Ton Jib Crane, an alloy chain sling was missing its intended safety latch; and c) At the Fab Production Area - the JLC Coffing 1 Ton Hoist, an alloy chain sling was missing its intended safety latch.
