29 CFR  1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):    a) Jobsite, 4 River Road, McKees Rocks, PA:  On or about March 1, 2023, employees were working on an unguarded steel plate 7.5 feet above the ground on top of the Red Bud machine to make adjustments to the coils being cut underneath. b) Jobsite, 4 River Road, McKees Rocks, PA:  On or about March 1, 2023, employees were exposed to fall hazards of approximately 1' into the Red Bud machine while it was operating, while they were standing, sitting or kneeling on the unguarded 72" x 108" steel plate that covers approximately one third of the opening on top of the machine.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Jobsite, 4 River Road, McKees Rocks, PA:  On or about March 1, 2023, employees were exposed to machine guarding hazards/caught in between hazards  while working on top of the Red Bud machine to make adjustments to the coils being cut underneath, while it was cutting the steel coils into sheets. b) Jobsite, 4 River Road, McKees Rocks, PA:  On or about March 1, 2023, employees were exposed to caught in between hazards at the shear blade adjustment area that was not fully guarded.
