On or about 2/27/2023, an employee was exposed to thrown from, pinned under, or crushed by hazards from a forklift, when the employer failed to ensure the employee utilize the restraint device when operating the equipment. The employee was operating the Yale Powered Industrial Truck, model ERP35VTN36TE082, serial number G807N08093N.
