29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8m) or more above the lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems:  347 Village Street, Bristol, Connecticut roofing project: On or about March 1, 2023, employees were exposed to a fall hazard of approximately 27 feet while performing residential roof repair activities on a 8/12 pitched roof without fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  347 Village Street, Bristol, Connecticut roofing project: On or about March 1, 2023, employees were using extension ladders to access the roof and the ladders did not extend at least 3 feet past the landing exposing them to a fall hazard of approximately 11 feet.
