29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). On or about March 2, 2023, at 3860 W Wisconsin Avenue in Grand Chute, Wisconsin, an employee was exposed to a fall hazard working from a Genie Z6034 Boom Lift 16 feet above surface level without using required personal fall protection equipment. The Alliance Construction and Design, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.453(b)(2)(v), which was contained in OSHA inspection number 1538141, citation number 1, item number 1 and was affirmed as a final order on August 16, 2021, with respect to a workplace located at S73 W16555 Janesville Road; Muskego, WI 53150.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. On or about March 2, 2023, at 3860 W Wisconsin Avenue in Grand Chute, Wisconsin, a Skyjack SJ 6826 RT scissor lift was not inspected before the work shift. Two console cables were damaged with broken protective covers and were wrapped in electrical tape.
