29 CFR 1926.416(a)(1): An employee was permitted to work in proximity to an electric power circuit that the employee could contact in the course of work and was not protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means. Location: 4 East 89th Street, meter room in basement On or about 2/24/2023, a) Employee was not provided with insulated tools to make live tap connections to the electrical bus bars energized with 250-volt AC. The 3-phase bus bars were not de-energized and grounded or effectively guarded.
