OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, when mobile elevated work platform was being used with damaged and rusted gearbox parts causing the gearbox to detach and descend rapidly, injuring an employee. a. Jobsite: 5949 Sadler Rd, Zellwood, FL, Harvesting Room 36: mobile elevated work platform was being operated with damaged rusted galled Hex Drive Shaft part # 4026032 and it goes into a Worm Shaft Coupler part # 4026564 and driven by a Gear Box part #4022484 , while harvesting mushrooms from heights of 4-feet 8-inches, exposing employees to fall and struck-by hazards, on or about February 23, 2023.
