29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) East side of the house - On or about March 2, 2023, the employer did not provide employees working from scaffolding below the roof with head protection, exposing the employees to struck-by hazards.  (b) East side of the house on the ground - On or about March 2, 2023, the employer did not provide the employee working from the ground cutting material with head protection, exposing the employee to struck-by hazards.

29 CFR  1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide.   (a) East side of the house - On or about March 2, 2023, the employer instructed the employees to build the scaffolding using a walkway plank that was 12 inches wide and not the required 18 inches exposing employees to fall hazards.
