29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. a) Receiving Departments/Aisles - On or about March 02, 2023, and at times prior, the employer exposed employees to trip hazards in that, the employer failed to ensure that boxes of merchandise were clear from the aisles when employees were walking/working and handling boxes of merchandise in the aisles. Products limited the width of the aisles 17 inches to 20 inches.

29 CFR 1910.26(c): Portable dock boards were not secured by anchoring them in place nor by using equipment or devices that prevented the dock board from moving out of a safe position. a) Transfer Department/Dock door S93 - On or about March 02, 2023, and at times prior, the employer exposed employees to struck-by hazards in that, the employer failed to ensure portable dock boards were secured from moving when employees were using the dock boards for moving and unloading the boxes of merchandise from the trucks every day.
