29 CFR 1926.404(f)(6):The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) At the Enclave at Bakers Farm Subdivision, Lot 125, 165 Amber Meadow, Braselton, Georgia on or about February 27, 2023, where workers performing framing operations were exposed to 110V electrical shock hazards in that the employer provided an orange extension cord that was frayed in multiple locations and was missing the ground pin. The cord was powering equipment including a DeWalt circular saws and a DeWalt Reciprocating saw.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) At The Enclave at Bakers Farm Subdivision, Lot 125, 165 Amber Meadow, Braselton, Georgia on or about February 27, 2023, where workers performing framing operations were exposed to approximately 29-ft. fall hazards from the 6/12 pitched roof in that the employer did not require the use of personal fall protection.

29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) At The Enclave at Bakers Farm Subdivision, Lot 125, 165 Amber Meadow, Braselton, Georgia on or about February 27, 2023, the employer exposed employees to struck-by hazards while operating circular saws and pneumatic nail guns, in that protective eyewear was not provided to employees engaged in framing activities. Monroy Framing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1551322, citation number 2, item number 1 and was affirmed as a final order on September 2, 2021, with respect to a workplace located at 28 Noble Oak Ct, LOT 28, Palmetto, Georgia 30268.
