29 CFR 1910.147(d)(5)(ii): Where there was a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or until the possibility of such accumulation: (a) Location - Clam Processing Room: On or about 3/2/2023, a supervisor was testing an energized conveyor belt after having installed a new belt and had not verified that there was no stored energy that could pose a hazardous level. As a result the supervisor's hand became caught in the belt and sprocket.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: (a) Location - Clam Processing Room: On or about 3/2/2023, a maintenance employee who was tasked with helping to install and test a new clam processing conveyor belt had not been trained on the company's Lockout/Tagout Procedures prior to working on equipment that required Lockout/Tagout procedures.
