29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). On or about March 1, 2023, and at times prior thereto, in the shop, employees conducted operations to prepare a mechanical perforating gun for service. The employer did not adequately assess the workplace to the determine the use of personal protective equipment to protect employees from struck-by and burn hazards created by the charge detonation blast.
