29 CFR 1926.350(a)(10):Oxygen cylinders in storage were not separated from fuel-gas cylinder or combustible materials (especially oil or grease), a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour. 1. Employees were exposed to explosion and burn hazards while working where two acetylene bottles were stored on the side of a bottle cage without a barrier to adjacent oxygen bottles. 2. Employees were exposed to explosion and burn hazards while working near a bottle cart contain an oxygen and acetylene fuel gas bottles that were not in use and not separated by a fire rated barrier.

1926.21(b)(2) The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Location: 1 Boston Wharf Road Boston, MA 02212 On or about March 3, 2023, Turner Construction did not fully document the instruction it provided to employees regarding the employer's procedure for prohibiting foot traffic on unfinished pan stairs.
