29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about March 1, 2023, and at times prior thereto, at the Phillips 66 Harvest Connection project site, employees prepared to install a new riser to the piping system from an unprotected side in a work platform and were not protected from falling approximately nine feet to the lower level.

29 CFR 1926.502(i)(4): All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard: On or about March 1, 2023, and at times prior thereto, at the Phillips 66 Harvest Connection project site, employees worked from an elevated platform which had an unidentified piece of plywood that was not marked with a warning word of "Hole", or "Cover" to indicate a hazard.
