29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net . On or about March 6, 2023, an employee was working from a roof without fall protection. This exposed the employee to a fall hazard greater than six(6) feet to the ground below.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On or about March 6, 2023, a platform being lifted by a powered industrial truck did not have a guardrail system that meet the requirements of paragraph (g)(4) and the employees in use of the platform were not in use of a personal fall arrest system. This exposed the employees to a fall hazard greater than six(6) feet to the ground below.
