Mulkearn Inc. dba Advanced Concrete Floor Finishes, Culinary area located within the site of 8500 Oliver Road, ERIE, PA 16509, on or about March 6th, 2023 and times prior thereto: Employees were working with and around corrosive chemicals including, but not limited to PPG brand Concrete Epoxy Primer Catalyst Comp B. Employees did not have access to a suitable facility for quick drenching or flushing of the eyes.

Mulkearn Inc. dba Advanced Concrete Floor Finishes, Culinary area located within the site of 8500 Oliver Road, ERIE, PA 16509, on or about March 6th, 2023 and times prior thereto: Employees were working with and around corrosive chemicals including, but not limited to PPG brand Concrete Epoxy Primer Catalyst Comp B. The company did not make arrangements for prompt medical attention for the jobsite in that, a suitable facility for quick drenching or flushing of the eyes and body was not available for immediate emergency use.
