29 CFR  1910.28(b)(1)(i):29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  On or about 3/6/23, employees of Patron Services LLC were exposed to unprotected sides or edges 4 feet or more above a lower level while repairing a pump jack.

29 CFR  1910.23(c)(2): 29 CFR 1910.23(c)(2): The employer did not ensure that each stepladder or combination ladder used in a stepladder mode is equipped with a metal spreader or locking device that securely holds the front and back sections in an open position while the ladder is in use.   On or about 3/6/23, employees of Patron Services LLC were exposed to falls due to using a step ladder that was not locked into an open position while the ladder was in use.
