29 CFR 1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work.  Location: 68 Gardner Road, Nahant, MA;  On or about 03/01/2023, employees were exposed to electrical hazards from working within 10-feet of live overhead electrical wires in the vicinity of the pumpjack scaffolding they erected and were working from on the jobsite.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration.  Location: 68 Gardner Road, Nahant, MA:  On or about 03/01/2023, the employer allowed a pumpjack scaffold to be erected by employees who did not exhibit adequate knowledge of training or experience and were not supervised or directed by a competent person, exposing all employees on-site to the following hazardous conditions:  1) Exposed to live overhead electrical lines within 10-feet of the aluminum scaffolding.  2) Exposure to fall hazards while working from an unguarded scaffolding platform from heights greater than or equal to 19-feet above a lower level.  3) Scaffolding not being secured to the structure by rigid triangular bracing at the bottom of the scaffolding (employees only installed triangular bracing at the top of the scaffolding).  4) Scaffolding not erected and bearing on base plates, mud sills, or other adequate hard surfaces.

Location: 68 Gardner Road, Nahant, MA;  On or about 03/01/2023, employees were allowed to utilize a portable extension ladder for the following purpose(s) for which it was not designed or intended:  1) as the workbench and top rail of the work platform of the pumpjack scaffolding.  2) as an improvised extension of the work platform at one end of the scaffolding where an employee was half sitting and standing on the pumpjack scaffold platform and on the rungs of the portable extension ladder.
