29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems. a) Worksite, 750 2nd Ave, Troy, New York, South Side second floor: On or about March 7, 2023, employees were removing the old conveyor from the south side of the second floor. Employees were not protected from falling through a hole approximately 10 feet to the first floor.
