On or about March 6, 2023 and at times prior to, employees worked from a residential roof without fall protection. Employees were exposed to falls of greater than 6 feet to lower levels.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about March 6, 2023, and at times prior there to, the side rails of an extension ladder did not extend at least 3 feet above the roof. Employees used the extension ladder to access the roof and were exposed to falls from the ladder.
