On 7 March 2023, several employees from Angel Secundino were performing work on a two-story roof using pneumatic nail guns without utilizing eye protection.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about 7 March 2023, ten workers from Angel Secundino were observed working on a two-story roof at 2300 West Clay Street in Saint Charles, Missouri without fall protection or with improper fall protection. They were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection, exposing them to falls to the ground below.
