29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person: (a)Alex Mejia at 310 North Maple Avenue, Hartford, SD 57033: On or about and at times prior to March 7, 2023, the employer did not ensure an accident prevention program was developed and implemented for the work environment to include regular inspections of the job-site, materials, and equipment; conducted by a competent person. Employees were conducting roofing operations on a multi-family residential property without the use of a fall protection system in place and lack of training. This condition exposed employees to fall hazards of greater than six feet.
