29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  On or about March 7, 2023, at a residential construction site located on 21307 Indigo Ruth in Spring, TX, employees were exposed to a fall hazard while performing masonry work more than 10 feet high from a scaffold that was not fully planked.

29 CFR  1926.451(b)(3):The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling.Note:  See exceptions in paragraphs (b)(3)(i) and (ii) of this section.  On or about March 7, 2023, at a residential construction site located on 21307 Indigo Ruth in Spring, TX, employees were working on the second level of a tubular welded frame scaffold where the front edge of all platforms was not 14 inches (36 cm) or less from the face of the work and no personal fall arrest systems was used, exposing the employees to a fall hazard of more than 10 feet.

29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about March 7, 2023, at a residential construction site located on 21307 Indigo Ruth in Spring, TX, employees were exposed to a fall hazard while working from a fabricated frame scaffolding without an access ladder or other safe means of access. 29 CFR  1926.451(g)(1)(vi):Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity).  On or about March 7, 2023, at a residential construction site located on 21307 Indigo Ruth in Spring, TX, employees were exposed to a fall hazard exceeding ten feet while performing masonry work from a scaffold without a fall protection system.
