29 CFR  1910.184(c)(2): Slings were shortened with knots or bolts or other makeshift devices.  Located in section 21 of the cemetery, a chain sling was wrapped around a mini-excavator tamper attachment and shortened with a makeshift device (a hook). Employees are exposed to material handling hazards including dropped loads and potential injuries such as broken bones or crushing.

Located in section 21, the chain sling used to lift grave covers into place was not protected from sharp edges of the excavator tamper. Employees are exposed to material handling and dropped load hazards that could result in broken bones or crushing injuries.

29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  1). Located in the mechanic's shop, a Rockwell Manufacturing Co. drill press was not guarded at the point of operation.   2). Located in the mechanic's shop, a yellow Enerpac hydraulic press was not guarded at the point of operation.   Employees are exposed to struck-by flying objects and caught-in hazards that could result in lacerations, eye injury, or amputations.

Located in the mechanic's shop next to the exit door, the yellow Enerpac hydraulic press was not securely anchored to the floor to prevent moving.  2). Located in the mechanic's shop next to the black cabinet, the Rockwell drill press was not securely anchored to the work bench to prevent walking or moving.   Employees are exposed to machine tipover hazards that could result in crushing or lacerations.

29 CFR  1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment.  Located in the mechanic's shop, a 120/208 volt safety shutoff switch was obstructed by a table and other objects. The shutoff switch powered electrical outlets including one connected to a welding machine.  Employees are exposed to an electrical hazard that can lead to injuries such as smoke inhalation or burns.
