29 CFR 1910.134(e)(6)(i): The employer did not obtain from the physician or other licensed health care professional (PLHCP) a written recommendation regarding the employee's ability to use the respirator:  a) On or about March 07, 2023, and at time prior, welding employees were required to use powered air purifying respirators (PAPRs) and mechanics were required to wear tight-fitting respirators when spraying primer and topcoat. The employer did not obtain a physician or other licensed health care professional (PLHCP) written opinion prior to employees being required to wear respirators. On March 21, 2023, the employer sent employees for fit testing but received only confirmation that their respirator questionnaire was complete and did not receive written recommendation from the PLHCP regarding the employees' ability to use the respirator.

29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT):  a) On or about March 07, 2023, and at times prior, employees required to wear tight-fitting respirators did not received a fit test either qualitative or quantitative before being assigned a respirator.  b) On or about April 24, 2023, and at times prior employees wearing half mask and full-face tight-fitting respirators were improperly given a fit test for a N95 filtering facepiece respirator while having facial hair.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve:  a) On or about April 24, 2023, and at times prior, one half-mask tight-fitting respirator and one full-face respirator were stored in a cabinet exposed to contamination and were not stored properly.
