29 CFR 1926.451(g)(1)(i):Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system; a) On March 8, 2023, located on the south side of a residential job site, an employee was exposed to a fall hazard installing siding onto an exterior structure while working from a ladder jack scaffold that was not protected from a fall hazard by the use of a personal fall arrest system. Potential fall height measured to be 13'-2".

29 CFR 1926.20(b)(2):Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. a) On March 8, 2023, located on the south side of a residential job site, an employee was exposed to a fall hazard installing siding onto an exterior structure while working from a ladder jack scaffold as there were no frequent and regular inspections performed by a competent person to ensure equipment such as a personal fall arrest system was being utilized in a safe condition. Potential fall height measured to be 13'-2".

29 CFR 1926.454(a):The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: a) The nature of any electrical hazards, fall hazards, and falling object hazards in the work area; b) The correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems being used; c) The proper use of the scaffold and the proper handling of materials on the scaffold; d) The maximum intended load and the load-carrying capacities of the scaffolds used; and e) Any other pertinent requirements of this subpart. c) a) On March 8, 2023, located on the south side of a residential job site, an employee was exposed to a fall hazard while working from a ladder jack scaffold in that the employer failed to train the employee by a qualified person to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.
