29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  (a) On March 7, 2023, three employee(s) were observed using a JLG boom lift to access the roof, two employees were without lanyards and therefore not anchored in the lift exposing them to a fall hazard of up to approximately twenty-four (24) feet to the ground below.  Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about March 7, 2023, the following employees were not trained to recognize the hazards of falling and procedures they could follow to minimize those hazards: (a) Three employees working to install roofing. (b) Four employees receiving roof trusses.
