On or about March 9, 2023, while performing roofing operations on a commercial building located at 2057 West 20th Street, Cleveland, Ohio 44113; the employer did not ensure all employees engaged in roofing activities on a low-slope roof with unprotected sides and approximately 8 feet or more above the lower level were protected from falling by use of a guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.
