29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  On or about March 9, 2023, while performing roofing operations on a commercial building located at 2057 West 20th Street, Cleveland, Ohio 44113; the employer did not ensure all employees engaged in roofing activities on a low-slope roof with unprotected sides and approximately 8 feet or more above the lower level were protected from falling by use of a guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift.  a) On or about March 9, 2023,  employees were observed while installing a roof on the 5th floor of a commercial building located at 2057 West 20th Street, Cleveland, Ohio 44113; the employer did not ensure that all employees working in an aerial lift were protected by a body belt and lanyard attached to the boom or basket.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  a) On or about March 9, 2023, employees were observed while installing a roof on the 5th floor of a commercial building located at 2057 West 20th Street, Cleveland, Ohio 44113; the employer did not ensure that the portable ladder was extended at least 3 feet above the upper landing surface being accessed.
