29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components thereof:  a. Bohl Equipment Company, at a worksite located at 534 West Laskey Road, Toledo, Ohio 43612: On or about February 8, 2023, the employer did not ensure that all employees were safely positioned or removed from testing area, when performing operational evaluations following the installation of a "Cascade Single/Double Attachment" onto a Yale forklift.  As the equipment was energized and controls engaged (shifting attachment from side to side testing for leaks), an employee operating in the proximity of a pinch point between the attachment and carriage was exposed to crushing hazards .
