29 CFR 1910.37(a)(4):  Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:  (a) 5450 Carlisle Pike, Mechanicsburg, PA 17050, Bdg 204 door K27 - On or about March 16, 2023, employees were exposed to trip/fall  hazards from the exit light emergency lighting  not function when tested.    (b) 5450 Carlisle Pike, Mechanicsburg, PA 17050, Bdg 402 bay D25 - On or about April 3, 2023, employees were  exposed to trip/fall  hazards  from the exit light that was hanging from one wire instead of being securely installed.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.305(g)(2)(iii):Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  (a) 5450 Carlisle Pike, Mechanicsburg, PA 17050, Bdg 201 at bay 4 - On or about March 16, 2023,  the jacketed flexible sheath on the power cord which powered the hand control Hi/Lo function on the Reel-O-Matic (labelled P1030) was cut back from the tension collar of the device which caused the tension of the cord weight to transmit to the terminal screws.   Abatement certification required within 10 days after abatement date. 29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm caused by crushed by  and struck by hazards created by unsafe utilization of racking systems:  (a) 5450 Carlisle Pike, Mechanicsburg, PA 17050,  Bdg 3 aisle 17; bdg 104; Bdg 10 bay 1 &2; Bdg 201; 403 slab - On or about March 9, 2023, March 16, 2023, and April 3, 2023, employees were used racking systems and Unicore stacking racks (nestainers) which had  visibly damaged on vertical and horizontal pieces.   (b) 5450 Carlisle Pike, Mechanicsburg, PA 17050, Bdg 204 - On or about March 16, 2023, employees were used a racking system with missing the locking pin on the top shelf which prevented  the top shelf  from being seated in place on the vertical post.   (c) 5450 Carlisle Pike, Mechanicsburg, PA 17050, Bdg 10 Aisle 21 and work area W1001; Bdg 11 - On or about March 9, 2023 and March 15, 2023, employees were used  racking systems which were not anchored to the floor at the footers.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.178(a)(4):  Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  (a) 5450 Carlisle Pike, Mechanicsburg, PA 17050,  Bdg 111 - On or about March 15, 2023, the employer did not ensure that employees operating the YALE High Reach material handling equipment and the LANSING High Reach material handling equipment using the in-house fabricated metal and wood boxes were doing so with the manufacturer's prior written approval to do so.   Notice 1, Item 6, 29 CFR 1910.178(a)(4) as required by 29 CFR 1960.8(b): The employer failed to obtain manufactures approval for additions to the powered industrial trucks. The employer was cited for a violation of the same standard on 07/03/2019, Inspection 1383346, date 08/6/2019 of final order and means of abatement.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. (b)  5450 Carlisle Pike, Mechanicsburg, PA 17050, Bdg 301 row D21 - On or about March 16, 2023, Employees were exposed to struck by hazards on the Behringer (#M8229) bandsaw from a lack of guarding on the unused portion of the blade.   (c) 5450 Carlisle Pike, Mechanicsburg, PA 17050, Bdg 403 bay 6 - On or about April 3, 2023, Employees were exposed to struck by hazards on the Do All (#M8230) bandsaw from a lack of guarding on the unused portion of the blade.
