29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation: a) Facility: On or about March 9, 2023, the employer did not complete the OSHA Form 300 for calendar year 2023 in the detail required in that work-related injury or illness entries did not include the location within the facility where the incident occurred. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
