Employees were engaged in warehouse activities at a shipping and receiving facility located at 850 Rundell Road, Gering, NE 69341, and were exposed to struck-by hazards in that.   A.	A powered industrial truck was not taken out of service when it was discovered that brakes could not sufficiently slow/stop the truck. B.	Several powered industrial trucks were not taken out of service when it was discovered that they did not have working horns, and,  C.	A powered industrial truck had not been taken out of service after the manufacturer installed carriage had been torn off while trying to pull another powered industrial truck out of the snow.
