29 CFR  1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8m) or more above the lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems.  Location - Groton Road, Chelmsford, MA.  On or about March 9, 2023, employees were not protected from falling to lower level in the following instances:  Instance A) Employees were not protected from falling while walking/working on third floor deck at a height greater than sixteen feet above lower level.     Instance B) Third floor. Two employees were exposed to four unprotected window and floor openings.  Instance C) Second floor. Employees were not protected from falling through unprotected window and floor openings.

29 CFR  1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.  Location - Groton Road, Chelmsford, MA.  On or about March 9, 2023, employees were exposed to falling while using a ladder that could be displaced.
