On or about March 9, 2023, employees were not protected from falling to lower level in the following instances: Instance A) Employees were not protected from falling while walking/working on third floor deck at a height greater than sixteen feet above lower level. Instance B) Third floor. Two employees were exposed to four unprotected window and floor openings. Instance C) Second floor. Employees were not protected from falling through unprotected window and floor openings.
