29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems:  At the workplace located at 900 Lower Lake Road, Saint Joseph, Missouri 64504, employees were exposed to trip and fall hazards, which include walking/working surfaces with broken grates and exposed openings.  1: On the third floor a 24" x 3" gap in the walking/ working surface near the entrance.  2: On the fourth floor an 18" x 2" hole in walking/working of the fourth floor near the entrance.
