Site - On March 9, 2023, an employee was critically injured after he fell approximately 34 feet through a floor hole that was not covered, guarded, or did not otherwise have employees in the area performing wood framing operations protected by personal fall arrest systems.

29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. Site - Employees performing wood framing operations were exposed to falls of approximately 30-35 feet when a guardrail was not present along the western edge of the uppermost floor and there were more employees present than there were alternate fall protection devices.
