29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Institutional structure located at 7 Quarry St. Gainesville, GA 30501, Roofing area: On or about 3/6/2023, and at times prior, the employer exposed employees to fall hazards of approximately 19-feet, 6-inch to the lower ground level, in that adequate fall protection system was not provided to employees engaged in shingle removal activities on an 8/12 pitched roof.

The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. a) Institutional structure located at 7 Quarry St. Gainesville, GA 30501, Roofing area: On or about 3/6/2023, and at times prior, the employer exposed employees to potential struck-by hazards, in that the fall protection system provided did not allow the attachment point to be in the center of the wearer's back near shoulder level, while engaged in shingle installation activities.
