On a 4:12 pitched roof of a temporary commercial work site, at 7002 Normandy Blvd. in Jacksonville, Florida 32205: On or about March 7, 2023, the employer exposed employees to fall hazards ranging from 12 feet 6-inches to 22 feet, in that fall protection was not utilized by all employees while performing roofing activities.
