29 CFR 1910.67(c)(2)(v): (v) A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, shall be worn and attached to the boom or basket when working from an aerial lift: a) 1101 Wheaton Ave, Millville NJ 08332: An employee working for the company was not using a personal fall arrest or travel restraint system while aloft in a JLG, Model 600 AJ aerial lift. Employee was approximately 30 feet above the ground thus exposing the employee to a fall hazard. Observed on or about /3/10/223. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.178(l)(2)(ii):The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace: 2) 1101 Wheaton Ave, Millville NJ 08332: Employee was observed operating a Toyota 5K serial #75418 powered industrial truck (PIT) without properly being trained or certified by the employer. Thus exposing employees to struck-by and caught-in-between hazards. Observed on or about 3/10/23. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
