29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) Jobsite located at 3151 Market Street, Philadelphia, PA.,- Southeast Side - The employer failed to protect employees performing work at or near an unguarded floor hole measuring approximately 5 feet in length, 2.5 feet in width, and 3 feet in depth; thus exposing employees to trip and fall hazards. Condition observed on or about March 10, 2023.
