29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about March 10, 2023 at 760 Flora Avenue, Akron, OH 44314 employees were exposed to caught in hazards in that the engines in the Shredder Engine Room were not guarded. Employees enter the room to turn on and off compressors with rotating shafts exposed.

29 CFR 1910.305(d)(2):Panelboards mounted in cabinets, cutout boxes, or enclosures designed for the purpose and were not dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons: On or about March 1, 2023 at 760 Flora Avenue, Akron OH 44314, the employer failed to ensure employees starting up equipment closed the dead front door. An employee was burnt on their hands after a motor being installed was shorted and created an arc flash upon being turned on due to the dead front door not being closed.
