29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. Location - 12 Oriole Dr, Andover, MA. On or about March 10, 2023, an employee was exposed to injury and a fall greater than 17 feet above ground level from a residential roof while walking/working where the personal fall protection system being used was attached to the front of employees waist not at center of employees back between shoulder blades.
