29 CFR 1910.243(b)(2): Air hose and hose connections used for conducting compressed air to utilization equipment was not designed for the pressure and service to which they were subjected: (a) On or about March 13, 2023, and at times prior thereto, in the Signal Metal Industries, Inc. facility, where pneumatic tools where connected to air hose lines with worm gear hose clamps that were not rated for the pressure they serviced and employees were exposed to struck by hazards.

29 CFR 1910.334(a)(2)(ii):There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made: (a) On or about March 13, 2023, and at times prior thereto, in the Signal Metal Industries, Inc. facility, where personnel were exposed to electric shock hazards when they used extension cords that had damage to the outer jacket, exposing the wires underneath, and the extension cord had not been removed from service.
